Early in our training, every doctor takes an oath rooted in an ancient principle: Primum non nocere—First, do no harm.
It is a noble compass. But somewhere along the arduous road through medical school, residency, and years of clinical practice, an unwritten rule takes over: Do no harm to anyone—except yourself.
From day one, medical education conditions us to treat our basic human needs as liabilities. We learn to ignore biological signals—pushing through 30-hour shifts, skipping meals, holding back tears, and brushing off physical and emotional exhaustion as mere badges of honor. Perfection is expected, error is condemned, and vulnerability is framed as a dangerous flaw.
We learn to treat our own minds with a level of neglect we would never tolerate for a patient.
This environment breeds a toxic mix of perfectionism and self-neglect. When distress or depression sets in, our default response is to double down—work harder, study longer, compartmentalize deeper. But human capacity has limits. When a physician’s mental health finally breaks, the fall is catastrophic because the culture gave them no tools to slow down.
Studies show that over half of all healthcare practitioners meet the criteria for burnout or a mental health diagnosis. Yet, the pervasive fear of disciplinary action, credentialing exposure, or losing a medical license forces doctors to keep running on an empty tank. We are dying under the weight of a system that demands infinite empathy for others while offering zero grace to the healer.
If we want to stop losing hundreds of physicians to suicide every year, we must change the rulebook. “First, do no harm” must begin in the mirror.
